Registration, Cancellation and Refund Policies:
Submittal of your on-line registration form will only reserve your child's place in the Hockey School. This submittal form must be followed within 2 weeks with a signed copy of the form along with a deposit equal to or greater than 50% of the tuition fee and a cheque for the balance postdated for June 1, 2010. This will confirm your child's place in the selected programme(s). Please make cheques payable to: CONCEPT HOCKEY INTERNATIONAL INC. and mail to:

Upon receipt of payment, confirmation will be mailed/e-mailed to you.
For cancellations before June 1, 2010 there will be a $50.00 fee for each session cancelled. No refunds will be made after June 1, 2010 except in the event of an accident or injury prior to or during the course. In this case, one half of the tuition fee will be refunded upon written and verbal verification by a physician. The other half may be used as a credit towards future CHI programmes or the whole amount may be used as a credit. Both of these are to occur within one year. No money will be refunded if this credit is not used or the person fails to show up. No refund cheques will issued after Sept 1, 2010.
CHI reserves the right to:
- Cancel or combine programs
- Adjust or combine groups as necessary
- Remove a participant from the school for disciplinary reasons. If a participant is removed, there will be NO REFUND forthcoming.
In consideration of the Participant and his/her parent being permitted to register the Participant, and participate in Concept Hockey International (CHI) Hockey Camps, do hereby forever release and discharge CHI it's directors, Agents, Employees and any person or Corporation connected herewith from all manner of action, injury, damages, costs, claims or demands which shall or may hereafter have, suffer or receive by reason of such participant in program on the campus. The release shall be binding on our heirs, assigns, executors and administrators. It is further agreed that CHI Hockey Camps do not and shall not be considered to guarantee or warrant such equipment as may be used in the conducting of said camp. CHI reserves the right to use any pictures or videos taken during the camp for instructional, advertising and/or promotional purposes.
Accident/Sickness Insurance:
All Students must have their own insurance plan.
